If you've been offered a faculty or staff position at The Media School, you'll need to fill out some paperwork before you can start work.
Please download, print and complete each form. Submit the completed forms by fax (812-856-1091) or in person to accounting associate Janet Laughlin, Bobbie Treadway or Sarah Gibson in Franklin Hall 330.
Because the forms require critical data, email is not a secure enough method of submission.
