Required forms and documents
You must complete and submit the following forms and agreements upon the timeline provided by the student services program coordinator in order to remain in good standing in your Media School travel program:
- Participation agreement
- Passenger information form

As outlined in your participation agreement, you must comply with all deadlines and communicate in a timely manner about all aspects of your travel preparation. It is your responsibility to notify the student services program coordinator about any changes to your travel plans as soon as possible.
